This website and our telehealth services are not intended for emergencies. If you are experiencing a psychiatric emergency, suicidal ideation, or any life-threatening situation, call 911 or go to your nearest emergency room immediately. You may also contact the 988 Suicide & Crisis Lifeline by calling or texting 988.
Use of this website, including submitting a contact form inquiry, does not establish a provider-patient relationship. A formal relationship is only established upon completion of intake paperwork, verification of eligibility, and confirmation of a scheduled appointment through the client portal.
All clinical services are delivered via a HIPAA-compliant telehealth platform. By engaging in telehealth services, you acknowledge and consent to:
Optic Integrated Care LLC is currently licensed to provide services to patients located in the state of Pennsylvania at the time of each visit. It is your responsibility to ensure you are physically located in a state where our provider is licensed at the time of your appointment.
Your protected health information (PHI) is handled in strict accordance with the Health Insurance Portability and Accountability Act (HIPAA). All records, communications, and billing are managed through our HIPAA-certified client portal. For full details, please review our Privacy Policy.
Optic Integrated Care LLC accepts select insurance plans. Coverage is not guaranteed until verified. You are responsible for any charges not covered by your insurance, including co-pays, deductibles, and non-covered services. Self-pay and cash-pay options are available. Contact us for current rates.
To provide the highest quality care and ensure availability for all patients, we ask that you carefully review our cancellation policy:
Patients are required to provide at least 24 hours' notice for any appointment cancellations or requests to reschedule. This allows us to offer the appointment time to another patient in need of care.
Appointments canceled with less than 24 hours' notice, as well as missed appointments ("no-shows"), will be subject to a late cancellation fee of $50. This fee is not typically covered by insurance and will be the patient's responsibility.
We understand that emergencies and unforeseen circumstances may arise. Fees may be waived at the provider's discretion in cases of documented emergencies or serious illness.
As this is a virtual practice, patients are responsible for ensuring they have a stable internet connection and are available at the scheduled time. Technical difficulties on the patient's end that prevent session completion may be considered a missed appointment.
If you arrive late to your virtual session, your appointment may be shortened to accommodate other scheduled patients. If you are more than 15 minutes late, the appointment may need to be rescheduled and may be treated as a missed visit.
All cancellation or no-show fees must be paid prior to scheduling future appointments.
By scheduling an appointment, you acknowledge and agree to this cancellation policy.
Controlled substance prescriptions are governed by federal and Pennsylvania state law. Prescriptions will only be issued following a proper clinical evaluation. Requests for early refills, phone-in prescriptions for controlled substances, or prescriptions outside the scope of an established relationship will not be honored.
